Zusammenfassung
Operationstechniken
In der HerniaSurge-Leitlinie besteht die höchste Evidenz hinsichtlich einer starken Empfehlung für netzbasierte Operationstechniken. Diese Evidenz gilt gleichermaßen für das Lichtenstein-Verfahren wie für die minimal-invasiven Verfahren TEP/TAPP. Bei diskret symptomatischen oder asymptomatischen Leistenhernien kann unter Berücksichtigung des Gesundheitszustandes und der sozialen Umstände „watchful waiting“ eine Option sein. Femoralhernien werden dagegen zeitnah mit Netzeinlage versorgt. Favorisiert werden zudem laparoendoskopische Techniken. Die Shouldice-Reparation erreicht von den Nahtverfahren die geringsten Rezidivraten und kann unter geeigneter Indikationsstellung, oder wenn der Patient keine Netzverstärkung wünscht, eine akzeptable Alternative darstellen. In diesem Fall sollte eine ausführliche Aufklärung erfolgt sein.
Netzwahl
Über das Komplikationspotenzial von Kunststoffnetzen sollte aufgeklärt werden. Das Flächengewicht gilt nicht mehr als geeigneter Parameter zur Klassifizierung von Kunststoffimplantaten und wird für die Netzauswahl nicht mehr empfohlen. Großporige (>1–1,5 mm), monofile Implantate besitzen das beste Integrationspotenzial und sollten eine Reißfestigkeit von etwa 16 Nm2 besitzen. Eine traumatische Netzfixierung wird nur noch bei großen medialen Hernien (M3-EHS) empfohlen. Primär nicht empfohlen werden Plug & Patch, doppelschichtige Kunststoffimplantate (z. B. PHS-System) oder andere dreidimensionale Devices, da hierdurch sowohl die anteriore wie auch die posteriore planare Schicht tangiert werden und die komplementäre Operationstechnik im Rezidivfall erschwert werden könnte. Zudem sind die höheren Kosten zu berücksichtigen.
Peri- und postoperative Aspekte
Perioperative Antibiotikaprophylaxe bei offenen Reparationsverfahren wird nur noch bei Patienten mit erhöhtem Infektionsrisiko empfohlen. Bei laparoendoskopischen Verfahren sollte eine Antibiotikaprophylaxe nicht erfolgen oder nur mit allergrößter Zurückhaltung eingesetzt werden. Eine sorgfältige Präparation reduziert chronische Leisten- und Hodenschmerzen. Bei Interferenz von Netz und Nerv kann dieser reseziert werden. Eine Rückkehr zur täglichen Aktivität wird innerhalb von 3 bis 5 Tagen empfohlen.
Qualitätssicherung
Die Dokumentation der Patientendaten sollte durch den Aufbau von Hernienregistern zur Qualitätssicherung und Entwicklung weiterer Therapieoptionen erfolgen. Die Implementierung der Leitlinien wird dabei von HerniaSurge unterstützt.
Abstract
Surgical techniques
The HerniaSurge guidelines have the highest evidence with respect to a strong recommendation for mesh-based surgical techniques. This evidence is equally valid for the Lichtenstein procedure as for the minimally invasive procedures TEP/TAPP. In the case of discrete symptomatic or asymptomatic inguinal hernias, watchful waiting can be an option, taking into account health status and social circumstances. Femoral hernias, on the other hand, should be treated promptly with mesh insertion. Also favored are laparoendoscopic techniques. The Shouldice repair achieves the least recurrences from the suturing procedures and may be an acceptable alternative when indicated or when the patient does not desire mesh reinforcement. In this case, a detailed patient education is necessary.
Mesh choice
The complication potential of plastic meshes should be explained. The weight is no longer considered a suitable parameter for the classification of meshes and is no longer recommended for mesh selection. Large pore (>1–1.5 mm) monofilament implants have the best integration potential and should have a tear strength of approximately 16 Nm2. Traumatic mesh fixation is only recommended for large medial hernias (M3-EHS). Primarily not recommended are Plug & Patch, double-layered plastic implants (such as the PHS system) or other three-dimensional devices, as this could affect both the anterior and posterior planar layers and complicate the complementary surgical technique in the event of recurrence. In addition, the higher costs have to be considered.
Perioperative and postoperative aspects
Perioperative antibiotic prophylaxis in open repair procedures is recommended only in patients with an increased risk of infections. In laparoendoscopic procedures, antibiotic prophylaxis should not be performed or used with the utmost restraint. Careful preparation reduces chronic inguinal and testicular pain. In the case of interference of mesh and nerve, the nerve can be resected. A return to daily activity is recommended within 3–5 days.
Quality assurance
The documentation of patient data should be done by establishing hernia registers for quality assurance and for the development of further treatment options. The implementation of the guidelines is supported by HerniaSurge.
