Abstract
Objective
This article describes the percutaneous technique of a minimally invasive basal closing wedge osteotomy for correction of hallux valgus.
Indications
This procedure allows correction of severe deformity with a minimally invasive approach.
Contraindications
No specific contraindication; a fusion would be preferred for an arthritic tarsometatarsal or metatarsophalangeal joint.
Surgical technique
The surgical technique is based on the use of burrs specifically adapted for foot surgery. A basal closing wedge osteotomy is performed and fixed percutaneously. Each step is controlled under fluoroscopy.
Postoperative management
A postoperative heel shoe is prescribed for 6 weeks with crutches. The foot is elevated during the first 2 weeks. Impact is forbidden for 3 months.
Results
The authors report good and excellent results with an average correction of the hallux valgus angle of 26° and an intermetatarsal angle of 8.2°.

Ziel
Im vorliegenden Beitrag wird die perkutane Technik einer miminal-invasiven basalen Closing-Wedge-Osteotomie zur Korrektur eines Hallux valgus beschrieben.
Indikationen
Dieses Verfahren ermöglicht die Korrektur schwerer Deformitäten mithilfe eines miminal-invasiven Ansatzes.
Kontraindikationen
Spezifische Kontraindikationen bestehen nicht, bei arthritischen tarsometatarsalen oder metatarsophalangealen Gelenken würde eine Fusion bevorzugt.
Chirurgische Technik
Die chirurgische Technik basiert auf der Verwendung von Bohrern, die speziell auf die Fußchirurgie ausgerichtet sind. Eine basale Closing-Wedge-Osteotomie wird perkutan durchgeführt und fixiert. Jeder Schritt wird unter Durchleuchtung kontrolliert.
Postoperatives Management
Ein postoperativ zu tragender Verbandsschuh und Gehstützen werden für 6 Wochen verordnet. Der Fuß wird in den ersten 2 Wochen hochgelagert. Belastung ist 3 Monate lang verboten.
Ergebnisse
Die Autoren berichten über gute und ausgezeichnete Ergebnisse bei einem durchschnittlichen Korrekturwinkel von 26° für den Hallux valgus und einem Intermetatarsalwinkel von 8,2°.
