Zusammenfassung
Grundlagen: Nach Einführung der Handgelenksarthroskopie zur invasiven Abklärung und Diagnostik unklarer Handgelenkschmerzen wird diese Operationstechnik zunehmend zur Behandlung von frischen intraartikulären Radiusfrakturen loco typico angewendet.
Methodik: Bei insgesamt 30 Patienten mit intraartikulären distalen Radiusfrakturen wurde eine arthroskopisch assistierte Frakturversorgung vorgenommen. Es handelte sich um 3 B-Frakturen und 27 C-Frakturen nach der AO-Klassifikation. Bei 23 dieser Patienten wurden behandlungswürdige karpale Zusatzverletzungen diagnostiziert. 23 von 25 Patienten wurden nach durchschnittlich 32,5 Monaten nachuntersucht und mittels des Scores nachCooney evaluiert.
Ergebnisse: 70% der Patienten zeigten ein exzellentes oder gutes und jeweils 15% ein befriedigendes bzw. schlechtes Ergebnis. Bei 2 Patienten waren Sekundäreingriffe erforderlich (1 Ulnaverkürzung, 1 Plattenosteosynthese des Radius).
Schlußfolgerungen: Die arthroskopisch assistierte Versorgung distaler intraartikulärer Radiusfrakturen ist nicht nur zur Evaluierung von karpalen Zusatzverletzungen hilfreich, sondern zeigt auch gute mittelfristige Ergebnisse.
Summary
Background: After the introduction of wrist arthroscopy for the invasive evaluation of chronic wrist disorders, this technique is increasingly used for the treatment of acute distal intraarticular radius fractures.
Methods: Arthroscopically assisted therapy was used in 30 consecutive patients suffering from distal intraarticular radius fractures. In particular, there were 3 fractures of type B and 27 fractures of type C, according to the AO-fracture classification system. In 23 patients, concomitant carpal injuries which needed additional treatment were diagnosed. patients were followed up after a mean period of 32,5 months and evaluated according to the score ofCooney et al.
Results: 70% of the patients revealed excellent or good results, in 15% each, we found moderate or poor results. In two patients, revision surgery was necessary (one ulna shortening osteotomy, one plate osteosynthesis of the radius).
Conclusions: Arthroscopically assisted treatment of distal intraarticular radius fractures is a valuable tool for the diagnosis of concomitant carpal injuries in case of intraarticular fracture pattern. Furthermore, the mid-term results of these technique seem encouraging.
