Letale postoperative Komplikationen beim kardialen Risikopafienten

Zusammenfassung

Die Pathogenese des postoperativen Verlaufs mit letalem Ausgang beim kardiovaskulären Risikopatienten wurde untersucht. Als wesentlicher Risikofaktor wurde die postoperative Anämie≤3,5 Mio. Ery/mm³ gefunden. Die Häufigkeit von Komplikationen und deren kombiniertes Auftreten in 68 letalen Fällen wurden überprüft. In der Mehrzahl der Fälle (57,5%) führte das Zusammentreffen von mindestens 3 verschiedenen Komplikationen zum Exitus. Die Häufigkeit dieser Kombinationen den Häufigkeiten der letzteren. Dagegen zeigte die Sepsis eine überdurchschnittlich hohe Tendenz, kombiniert mit Anämie, Thromboembolie und Pneumonie aufzutreten. Auf Grund der Ergebnisse wird beim kardiovaskulären Risikopatienten die routinemäßige präoperative Anwendung von Digitalisglykosiden empfohlen und die kombinierte Prophylaxe von Myokardinfarkt, Thromboembolie, Nierenversagen und Pneumonie bzw. Sepsis diskutiert.

Summary

Postoperative complications leading to death and their coincidence were analyzed in 68 patients with high cardiovascular risk. Anemia≤3,5 mio erythrocytes/mm³ during the postoperative period was found to be a main dangerous factor.

At least 3 complications together led to death in 57,5% of the cases. When 2 or more different complications occurred in 1 patient, the frequency of these combinations was related to the frequency of the single components, when cardiovascular complications were involved. Sepsis on the other hand was found at a higher rate in connection with anemia, thromboembolism and pneumonia.

As a consequence of the results routine proeperative digitalization is recommended for all patients with high cardiovascular risk and combined prophylaxis of myocardial infarction, thromboembolism, renal insufficiency and pneumonia or sepsis respectively is discussed.
