Résumé

Le système de soins peut être caractérisé entre autres par la multiplicité des agents (institutions et individus) qui agissent au nom de finalités concurrentes, voire contradictoires. L'objectif principal de l'organisation économique du système de soins est d'exercer une régulation qui assure la coordination des innombrables décisions des agents décentralisés pour qu'elles s'inscrivent de façon économique dans une finalité commune. Les mécanismes du marché appliqués au domaine de la santé sont incapables d'opérer la régulation nécessaire et la planification centralisée est source d'inefficacité. Il est donc nécessaire d'utiliser la voie de la régulation économique décentralisée; dans cette perspective les modes de financement de l'activité sanitaire sont essentiels et peuvent exercer une influence positive sur le comportement des agents et sur les coêts. L'information sur les clientèles hospitalières est une contribution importante qui doit permettre d'augmenter la transparence du fonctionnement du système de soins et faciliter la concurrence.

Das Gesundheitswesen ist unter anderem durch die Vielfalt der beteiligten Partner (Institutionen und Individuen) charakterisiert, welche letztlich unterschiedliche, möglicherweise sogar widersprechende Ziele verfolgen. Hauptaufgabe der ökonomischen Organisation des Gesundheitswesens ist sicherzustellen, dass die unzähligen Einzelentscheide aller dezentralisierten Partner so koordiniert werden, dass aus ökonomischer Sicht ein gemeinsames Ziel verfolgt wird. Die Marktmechanismen im Gesundheitswesen scheinen die Anwendung der notwendigen Regulierungen nicht zu erlauben, und zentrale Planung kann zu mangelnder Effizienz führen. Aus diesem Grund sind dezentralisierte Lösungen anzustreben. Aus dieser Sicht sind verschiedene Finanzierungsarten im Pflegebereich wichtig und können einen positiven Einfluss auf das Verhalten der Beteiligten und die Kosten ausüben. Information über die Patientenzusammensetzung der Spitäler ist ein wichtiger Beitrag, der erlauben soll, Transparenz ins Funktionieren des Pflegesystems zu bringen und Konkurrenz zu erleichtern.

Summary

The health care system can be characterized, among other facts, by the multiplicity of involved actors (institutions and individuals) acting for final goals in competition, or even in opposition. The principal goal of the economic organization of the health care system consists in applying a regulation to grant the coordination of the great number of decisions taken by decentralized actors in order to introduce them in an economic way into a common final goal.

The market forces appear unable to realize the necessary regulation in the health care system and centralized planning can lead to lack of effectiveness. For this reason, it's necessary to make use of a decentralized economic regulation; in this view health care financing systems are essential and could positively influence the behaviour of the actors as well as the costs.

Information on hospital Casemix represents an important contribution to better transparency of the working of the health care system and to facilitate competition.
