Extending the duration of the pill-free interval during combined oral contraception

Abstract

The effect of increasing the duration of the pill-free interval from 7 to 9 and 11 days during the first 4 months of combined oral contraceptive therapy was studied by hormonal, ovarian follicle and mucus changes. Increasing the pill-free interval allowed more follicular development and more estradiol production. In some cases, pre-ovulatory values were achieved although ovulation did not occur. No differences could be detected between different types of combined oral contraceptive, including monophasic, triphasic, levonorgestrel-containing and gestodene-containing. The variation between individuals in the group of 28 was far greater than any variation in dose.

Resumé

On a étudié pendant les 4 premiers mois d'un traitement contraceptif oral combiné l'effet d'une prolongation de la durée d'intervalle entre la prise de pillules, de 7 à 9 et 11 jours, en fonction des changements hormonaux, du follicule ovarien et du mucus. Le prolongement de l'intervalle sans pillule a entraîné un développement folliculaire plus important et une plus grande production d'oestradiol. Dans certains cas les niveaux préovulatories ont été atteints, mais sans que l'ovulation ne se produise. Aucune différence n'a pu être décelée entre les différents types combinés de contraceptifs oraux, y compris les monophasiques, les triphasiques, ceux qui contenaient du lévonorgestrel et ceux qui contenaient du gestodène. Les variations entre les différents sujets d'un groupe de 28 femmes étaient beaucoup plus sensibles que n'importe quelle variation de dose.

Resumen

Se estudió durante los primeros 4 meses de un tratamiento anticonceptivo oral combinado el efecto de la prolongación de la duración del intervalo entre la toma de píldoras, de 7 a 9 y 11 días, en función de los cambios hormonales, del folículo ovárico y del moco. La prolongación del intervalo sin píldora permitió un desarrollo folicular mayor y una producción más alta de estradiol. En algunos casos se alcanzaron los niveles preovulatorios, pero sin que se produjera la ovulación. No pudo observarse diferencia alguna entre los distintos tipos combinados de anticonceptivos orales, inclusive los monofásicos, los trifásicos, los que contenían levonorgestrel y los que contenían gestoden. Las variaciones entre los diferentes sujetos de un grupo de 28 mujeres eran mucho mayores que cualquier variación de la dosis.
