140. Gefäßchirurgische Intervention bei tiefer Infektion nach Gefäßoperation

Summary

From 1969 to 1984, 3,298 vascular surgical interventions were performed. In 2%, a deep wound infection developed. Twelve patients had been preoperated on externally and were referred to our clinic with an infection. Of a total of 80 patients operated on, 26 died and 26 had to have amputations. In suitable cases, an attempt can be made by local aggressive therapy to save the implanted material and thus preserve function. Accompanying measures are: omentum transposition, muscle graft, and transposition to muscles. If the prosthesis has to be explanted, extraanatomical bypass is the method of choice in ischemic syndrome cases.

Zusammenfassung

Von 1969–1984 wurden 3298 gefäßchirurgische Eingriffe durchgeführt. In 2% kam es zur tiefen Wundinfektion. 12 Patienten waren extern voroperiert und wurden mit Infektion überwiesen. Von insgesamt 80 operierten Patienten verstarben 26, 26 mußten amputiert werden. In geeigneten Fällen kann durch lokale aggressive Therapie versucht werden, das implantierte Material und damit die Funktion zu erhalten. Flankierende Maßnahmen sind die Omentumtransposition, Muskelplastik und Versenkungsoperation. Muß die Prothese explantiert werden, stellt beim Ischämiesyndrom der extraanatomische Bypass das Mittel der Wahl dar.
