Zusammenfassung
Die Stabilität von fünf verschiedenen Antibiotika in vier Standardinfusionslösungen wurde mittels des Agardiffusionsverfahrens geprüft. Dabei konnte eine deutliche Inaktivierung der drei untersuchten Penicilline während 24stündiger Aufbewahrung bei Zimmertemperatur in allen Infusionen, besonders in Glukose- und Laevuloselösungen, nachgewiesen werden. Am ausgeprägtesten waren die Verlustraten beim Ampicillin, gefolgt von Penicillin G und Carbenicillin. Relativ stabil verhielten sich Cephalotin und besonders Gentamycin. — Die gleichzeitig bestimmten pH-Werte in den Infusionslösungen deuten auf die erhebliche, unphysiologische Abweichung der Wasserstoffionenkonzentrationen vom Neutralpunkt hin, was als eine der wesentlichen Ursachen für den Wirkungsverlust der Penicilline infolge hydrolytischer Spaltung und Polymerisation zu werten ist. — Aufgrund der vorliegenden Ergebnisse wird empfohlen, Antibiotika nur in Kurzinfusionen resp. Bolusinjektionen zu applizieren.
Summary
The stability of five different antibiotics in four standard infusion solutions was tested by means of the diffusion method. After standing for 24 hours at room temperature a distinct inactivation of the penicillins tested could be seen in all solutions, especially glucose and levulose. The rate of loss of action of ampicillin was the most noticeable followed by penicillin G and carbenicillin. The behaviour of cephalotin was relatively stable and that of gentamicin was even more so. pH values of the solutions determined at the same time indicated a considerable, non-physiological deviation from the neutral on the part of the hydrogen ion concentrations. This can be taken as one of the fundamental causes for the loss of action of penicillin, resulting from hydrolytic fission and polymerisation. On the basis of the above results it is recommended that antibiotics be administered in short infusions or bolus injections only.
