Summary
After a short note on topography and a review of the literature the surgical removal of the dens epistropheus by atransoral—transpharyngealapproach is described. The surgical and post-operative difficulties are mentioned. We believe this procedure enables one to remove the dens epistropheus without injury to the cervical part of the medulla. Indications for this procedure include platybasia and dislocated fractures.
Zusammenfassung
Nach Darlegung der anatomischen Verhältnisse und einer Literaturübersicht wird der transorale-transpharyngeale operative Zugang zum Clivus, zum atlanto-axialen Übergang und zu den oberen drei Halswirbelkörpern am Beispiel einer Densexstirpation bei basilärer Impression beschrieben. Der Operationsverlauf wird eingehend dargestellt. Die in der Literatur angegebenen Komplikationen werden angeführt, die in Hinsicht auf die technischen Schwierigkeiten der Naht des oberen Drittels des Schleimhautschnittes mit unseren Erfahrungen übereinstimmen. Die Eröffnung des retropharyngealen Raumes bedeutet bei gleichzeitigem hohen Antibioticaschutz keine übergroße Infektionsgefahr. Gleichzeitig werden die möglichen Indikationen des ventralen Zuganges zum oberen Pol der Wirbelsäule und zum vorderen Hirnstamm diskutiert. Gegenüber einem dorsalen Vorgehen ist der Operationsweg von ventral übersichtlicher und die Möglichkeit der intraoperativen Markschädigung wesentlich geringer. Weiterhin entfällt bei disloziertem Dens die Gefahr der lagerungsbedingten Halsmarkschädigung.
