Polyneuroradiculitis Guillain Barré- and Fisher-syndrome in cytomegalovirus infections

Summary

In seven patients suffering from polyneuroradiculitis of the Guillain-Barré type and one case of Fisher syndrome, a cytomegalovirus infection was found. The elevation of the virus-specific IgM-antibodies was confirmed in all cases. The clinical course was severe and acute with respiratory disturbances, but complete remission occurred within 1–4 months.

The complement fixing antibodies were highest during the first week of illness, so that further elevations could not be demonstrated, therefore making the IgM-antibody method of special importance. The role of cytomegalovirus infections in polyneuroradiculitis is also discussed.

Zusammenfassung

Wir berichten über 7 Fälle mit Guillain-Barré Syndrom und einen Fall von Fisher-Syndrom, bei denen eine Cytomegalie-lnfektion serologisch durch Bestimmung der virusspezifischen IgM-Antikörper diagnostiziert werden konnte.

Die neurologischen Symptome entwickelten sich akut, anfangs mit schweren Paresen und Atemstörungen, aber voller Remission in 1–4 Monaten.

Serologisch zeigten sich maximale Titer der Cytomegalie-Komplementbindungsreaktion bereits in der ersten Krankheitswoche, so daß weitere Titeranstiege nicht mehr beobachtet wurden, während die IgM-Antikörper auch noch in der 4.–5. Woche ansteigen konnten. Daher ist diagnostisch die IgM-Antikörpermethode von besonderer Bedeutung.

Cytomegalie-Infektionen sind bei akuter Polyneuroradikulitis offenbar häufiger als bisher angenommen.
