Summary
It is generally suspected that oral contraceptives cause circulatory disturbances. In particular it is discussed wether or not obliterations of cerebral vessels are found under the influence of these drugs. The investigations and results which have been published so far for different reasons are not reliable as to this question. Therefore the problem was reviewed regarding our own material. The methodological way could be prevailingly retrospective for the presence.
From a total of 750 patients suffering from generalised cerebral circulatory disturbances and being treated in the ward 120 cases were collected in which stenosis or occlusion of cerebral arteries could be shown by angiography or autopsy. Within this group no statistically significant predominance of female patients was found. However, the problem of the small number of cases is critically to be discussed. In respect to our findings it is supposed that oral contraceptives in spite of their frequent application do not influence particularly the cerebral circulation. In this context it should be mentioned that a rather constant representation of younger women (ages between 15 and 45 years) is included in our material.
But the problem seems to be a more complex one. Thus, the correlation of the frequency of cerebrovascular processes in pregnancy (childbed included) as compared to that under oral contraceptives was intensively discussed. All previous publications concerning the problem of a possible or probable connection between these conditions and cerebrovascular insufficiency only refer to this questions but do not present exact figures. The methodological deficiency, however, do not permit to exclude either a positive or a negative correlation. To our feeling this problem can only be solved by widespread prospective investigations.
Zusammenfassung
Da der hormonellen Antikonzeption u. a. auch ein ursächlicher Faktor beim Entstehen von Gefäßprozessen ganz allgemein zugesprochen wird, wurde der Frage nachgegangen, ob obliterative Veränderungen an den cerebralen Gefäßen ebenfalls betroffen seien. Da die Mitteilungen zu dieser Frage in der Literatur aus verschiedensten Gründen nicht befriedigen konnte, stellte sich die Aufgabe, am eigenen Krankengut dieser Problematik nachzugehen. Der methodische Weg konnte zunächst überwiegend nur retrospektiv sein.
In die Studie wurden von 750 an einem cerebralen Gefäßprozeß Erkrankter nur die 120 Patienten aufgenommen, bei denen eine Stenose oder eine Obliteration der betroffenen Arterie oder Vene angiographisch oder autoptisch nachgewiesen werden konnte. Somit stand ein Krankengut klarer Definition zur Verfürgung. Nach diesem besteht kein statistisch gesicherter Unterschied in der Geschlechtsverteilung, wobei allerdings das Problem der kleinen Zahl kritisch gewürdigt werden muß. Allein auf Grund dieses Befundes konnte geschlossen werden, daß den hormonellen Antikonzeptiva trotz weiter Verbreitung keine pathogenetische Rolle bei der Entstehung cerebraler Gefäßprozesse zukommt, besonders auch dann, wenn sich eine ziemlich konstante jährliche Repräsentation der jüngeren Patienten (bis zum 44. Lebensjahr) nach den vorliegenden Ergebnissen abzeichnet. Die Problematik aber ist komplexer. Daher wurde die Beziehung der Häufigkeit cerebrovasculärer Prozesse und Schwangerschaft (einschließlich Wochenbett) oder oraler Kontrazeption erörtert. Es konnte dargestellt werden, daß alle bisherigen Arbeiten zur Frage des möglichen oder wahrscheinlichen Kausalzusammenhanges zwischen hormonellen Schwangerschaftsverhütungsmaßnahmen und Auftreten cerebrovasculärer Prozesse zwar Einblicke in diese Problematik vermitteln, daß sie aber letztlich aus methodischen Gründen nicht im positiven oder negativen Sinne einer Korrelation gewertet werden können. Aus diesem Grunde werden Vorschläge unterbreitet, nach denen nur großangelegte prospektive Beobachtungsreihen diese Frage einer Lösung näherbringen können.
