Surgical correction of idiopathic medial femoral torsion

Summary

We reviewed 28 hips in 14 patients with severe bilateral idiopathic medial femoral torsion which had been corrected by intertrochanteric rotation osteotomy using crossed pins for fixation and a hip spica. There were 10 girls and 4 boys with a mean age of 7 years and 9 months. All had functional and cosmetic disability before operation. The average follow up was for 3 years and 9 months. Full correction was achieved in 27 hips. In one partial avascular necrosis of the femoral head occurred one year after operation and was treated by an acetabular augmentation procedure with a satisfactory outcome.

Résumé

Les auteurs présentent les résultats cliniques de 28 ostéotomies chez 14 patients ayant une torsion interne idiopathique bilatérale des fémurs. Il s'agit d'ostéotomie inter-trochantérienne de rotation avec fixation par broches croisées et plâtre Spica. Cette étude a inclu 10 filles et 4 garçons de 3 à 11 ans, avec une moyenne de 7 ans et 9 mois au moment de l'opération. Avant l'opération, tous souffraient d'une fonction diminuée et des anomalies esthétiques. Le délai d'observation a été en moyenne de 3 ans et 9 mois et tous les patients ont été suivis jusqu'à consolidation. Pour 27 hanches, la correction a été complète, sans complication post-opératoire. Dans 1 cas, une nécrose avasculaire de la tête fémorale a été observée 1 an après l'opération. Elle a été traitée par une butée osseuse avec actuellement un résultat satisfaisant. Au total l'intervention s'est avérée efficace et appropriée pour la correction de la torsion fémorale sévère chez des enfants de plus de 8 ans.
