Das Pneumophon

Zusammenfassung

Bringt man einen konstanten Ton durch Knochenleitung oder Luftleitung ans Ohr und läßt dann den Luftdruck im äußeren Gehörgang von positiv nach negativ sinken, dann kann eine bestimmte Zone des deutlichsten Hörens (Z.D.H.) festgestellt werden. Nach diesem Prinzip wurde für klinische Zwecke ein, einfacher Apparat, “das Pneumophon” konstruiert. Es ergab sich, daß die Z.D.H. bei normalen Ohren sich von +2 bis −2 cm Wasser erstreckt. Bei Tubärkatarrh, nach Kehloperationen usw. ist die Z.D.H. negativ, z. B. −16 bis −20. Direkt nach Luftinsufflation, und zwar bevor der Patient geschluckt hat, findet man meistens eine stark positive Z.D.H. bei +10 bis +14. In der Z.D.H. hat man also ein Mittel zur quantitativen Diagnose des Tubaverschlusses und eine Kontrolle des Effekts der Insufflation. Bei Trommelfellperforation und Stapesankylose kann keine Z.D.H. festgestellt werden. Für die Diagnose der Stapesankylose verdient die Untersuchung mit dem Pneumophon den Vorzug vor den unsicheren Ergebnissen derGelléschen Probe.

Si l'on transmet un son constant à l'oreille, par conduction osseux ou par voie aérienne et qu'on abaisse la pression de l'air dans le conduit auditif externe, de sorte que de positive elle devient négative, on peut déterminer une certaine zone de bonne acuité auditive. C'est, en se basant sur ce principe, qu'on a construit pour usage clinique un appareil simple; le pneumophone! Il se trouve que chez les sujets à oreilles normales, la zone de bonne acuité auditive s'étend entre +2 et −2 cm. d'eau. En cas de catarrhe tubaire, après des opération du pharynx, etc. la zone est négative, s'étendant p.e. entre −16 et −20 cm. d'eau. Immédiatement après l'insufflation tubaire on trouve une zone de bonne acuité auditive positive, s'étendant p. e. entre −10 et −14 cm. d'eau. La zone de bonne audition peut donc servir à établir le diagnostic quantitatif de l'obstruction tubaire et à contrôler l'effect de l'insufflation. En cas de la perforation du tympan et de l'ankylose de l'étrier aucune zone peut être déterminer. Pour le diagnostic de l'ankylose de l'étrier il faut préférer l'examen à l'aide du pneumophone à l'épreuve deGellé aux résultats peu sûrs.

Summary

When a constant sound by means of bone or air conduction is brought to the ear, it is possible, if the air pressure in the meatus externus is lowered gradually from positive to negative, to mark off a special Zone of Distinct Haering (Z.D.H.). On this principle a small apparatus was build for clinical purposes, the Pneumophone! It appeared that the Z.D.H. in normal ears extends from +2 to −2 cm. water pressure. In the case of a catarrh of the tuba Eustachii, after a tonsillectomia etc. the Z.D.H. was found to be negative, f.i. −16 to −20. Directly after air insufflation before swallowing in most cases a strongly positive Z.D.H. was found f.i. +10 to +14. Therefore the Z.D.H. is a means to the quantative diagnosis of the tuba insufficiency and a control on the effect of the insufflation. In the case of perforation of the eardrum and in the case of stapes-ankylosis, there is n't a Z.D.H. at all. For the diagnosis of the stapes-ankylosis the investigation with the pneumophone must be preferred to the unreliable results of the test ofGellé.
