Zusammenfassung
Die lebenslang erforderliche Immunsuppression nach solider Organtransplantation bedingt eine Anfälligkeit der Organempfänger für bakterielle, virale und fungale Infektionen. Eine fehlende Leukozytose oder mitigierte klinische Symptomatik schließen eine schwere, behandlungsbedürftige Infektion bei transplantierten Patienten nicht aus. Während in der Frühphase nach Transplantation vor allem typische nosokomiale und opportunistische Infektionen eine Rolle spielen, entspricht das Erregerspektrum der Infektionen mehr als 6 Monate nach der Organtransplantation im Wesentlichen dem anderer Patienten. Zurückliegende Nachweise von multiresistenten Keimen als auch Arzneimittelinteraktionen mit Immunsuppressiva (insbesondere Cyp3A4-Inhibitoren und -Induktoren) sind bei der Auswahl der Antibiotika und Antimykotika zu berücksichtigen. Schwere Infekte als auch elektive oder Notfalloperationen erfordern je nach immunsuppressivem Regime eine Anpassung der immunsuppressiven Medikation.
