Zusammenfassung
Die nicht-alkoholische Fettlebererkrankung (NAFLD) wird in der westlichen Welt zunehmen, Prognosen gehen von einer Verdoppelung der NASH-Zirrhosen bis ins Jahr 2030 aus. Bis heute sind in Deutschland keine Versorgungsstrukturen etabliert und weltweit sind keine Medikamente zur Behandlung von Patienten mit NAFLD zugelassen. FDA und EMA haben die Identifizierung von NASH-spezifischen Medikamenten daher zu einem wesentlichen „unmet medical need“ erklärt. Die Weiterentwicklung der Screening-Algorithmen und deren Integration in die Versorgungsstrukturen sind eine erste wichtige Voraussetzung für zukünftige Therapiestrategien. Neue Therapieansätze, die derzeit in klinischen Studien untersucht werden, sind bereits sehr vielversprechend. Der wichtigste Aspekt ist dabei die signifikante Reduktion der prognostisch relevanten hepatischen Fibrose. In klinischer Entwicklung sind derzeit unter anderem Agonisten nukleärer Hormonrezeptoren (FXR- und PPAR-Agonisten), Chemokinrezeptor-Inhibitoren, Agonisten des Schilddrüsenhormonrezeptors β(THR-β) sowie Analoga enterohepatischer Hormone (GLP-1 und FGF 21) und SGLT2-Inhibitoren. Es bleibt zu klären, inwiefern alle Patienten mit relevanter NASH-Fibrose (F2 und 3) oder nur die Patienten mit dem unmittelbar höchsten Progressionsrisiko zur Zirrhose (F3) zukünftig eine spezifische medikamentöse Therapie erhalten sollten. Die Obeticholsäure ist als einziges Medikament mit einem signifikanten Benefit in der Phase-3-Interimsanalyse der primäre Kandidat für die erste konditionale Zulassung.
