Klassifikation und medikamentöse Therapie des Nierenzellkarzinoms

Zusammenfassung

Nierenzellkarzinome (NZK) machen 2–4 % der malignen Neoplasien im Erwachsenenalter aus. Bei Diagnosestellung liegt häufig ein lokalisiertes Stadium vor. Liegt ein metastasiertes NZK vor, das einer Lokaltherapie nicht zugänglich ist, rücken medikamentöse Verfahren in den Vordergrund. Durch zahlreiche Neuzulassungen hat sich die Therapielandschaft hierbei in den vergangenen Jahren grundlegend gewandelt. Neben der zielgerichteten Therapie mit Tyrosinkinase-Inhibitoren werden Immuncheckpoint-Inhibitoren sowie Kombinationstherapien aus beiden Substanzgruppen eingesetzt. Beide Behandlungsformen gehen auch weiterhin mit einer hohen Rate an Nebenwirkungen einher. Diese lassen sich durch ein gut abgestimmtes Nebenwirkungsmanagement in der Regel gut behandeln. Der Artikel gibt einen Überblick über gängige Klassifikationssysteme sowie aktuelle medikamentöse Behandlungsstrategien für das NZK.
