Zusammenfassung
Extraintestinale Manifestationen bei CED sind häufig und kommen v. a. im Bereich der Gelenke, der Haut, der Augen und der Gallenwege vor. Ihre Diagnose und adäquate Therapie ist daher ein entscheidender Bestandteil der Behandlung von Patienten mit CED. Obwohl für das Kollektiv der CED-Patienten große kontrollierte Studien meist fehlen, so bestehen in der Regel doch klare und erfolgreiche, von anderen Krankheitskollektiven extrapolierte oder durch ausreichende klinische Erfahrung gestützte Therapiestrategien. Manche EIM, wie die peripheren Arthralgien/Arthritiden, das Erythema nodosum und die Episkleritis, zeigen eine Assoziation zur Aktivität der CED und sprechen daher auf eine Behandlung der Grunderkrankung an. Andere EIM, wie die axiale Spondylarthropathie, das Pyoderma gangraenosum, die Uveitis und die primär sklerosierende Cholangitis verlaufen in der Regel unabhängig von der Grunderkrankung und bedürfen einer eigenständigen Therapie. Die adäquate Diagnostik und Therapie von EIM bedarf einer besonderen Erfahrung im Bereich der CED und insbesondere bei selteneren und nicht-CED-schubassoziierten Erkrankungen eines interdisziplinären Diskurses zwischen Gastroenterologe, Rheumatologe, Dermatologe und Augenarzt.
CED-assoziierte Erkrankungen, v. a. die Anämie und Osteoporose, sind häufig. Sie müssen verhindert bzw. adäquat therapiert werden. In der Regel besteht ein enger Zusammenhang mit Aktivität und Schweregrad der Grunderkrankung. Die adäquate Therapie der CED ist daher auch hinsichtlich der assoziierten Erkrankungen grundlegend für eine erfolgreiche ärztliche Versorgung. Zusätzlich erfolgt die spezifische Therapie der assoziierten Erkrankung, z. B. durch Substitution von Eisen oder Vitamin B12 bei der Anämie und eine Behandlung der Osteoporose mit Bisphosphonaten. Bei Patienten mit M. Crohn sollte ein Gallensäureverlustsyndrom erkannt werden, die Behandlung verhindert auch eine vermehrte Bildung von Gallen-und Nierensteinen. Wie auch bei anderen entzündlichen Erkrankungen steigt bei Patienten mit aktiver CED das Risiko für Thrombosen und die Notwendigkeit einer adäquaten Thromboseprophylaxe.
