Zusammenfassung
Die Standardtechnik der extraperitonealen Patchplastik (TEP) sollte entsprechend der Empfehlungen in den Guidelines der internationalen Fachgesellschaften vorgenommen werden. Die Details der Standardtechnik der TEP werden in diesem Kapitel besprochen. Spezielle Risiken bei der TEP stellen die Operation von beidseitigen Leistenhernien, Rezidivleistenhernien, Skrotalhernien, inkarzerierten Leistenhernien, Leistenhernien nach vorausgegangenen Operationen im Unterbauch und im kleinen Becken und Aszites, Leistenhernien bei Patienten mit Koagulopathie oder antithrombotischer Therapie sowie im höheren Alter dar. Hier gilt es, insbesondere die Indikation zu einem endoskopischen Verfahren unter Abwägung der Risiken und des Nutzens für den Patienten zu stellen. Der Prävention von Komplikationen wird in diesem Kapitel besondere Aufmerksamkeit geschenkt. Aber auch die Behandlung von eingetretenen intra- und postoperativen Komplikationen wird ausführlich dargestellt. Bei sorgfältiger Beachtung der dargestellten Standardtechnik der TEP, ihrer Risiken und der Strategien zur Komplikationsprävention ist dem Leser das theoretische Rüstzeug an die Hand gegeben, die TEP mit einer niedrigen Komplikationsrate in der Praxis umzusetzen.
