Die klassische Sexualtherapie: Grundlagen und Entwicklungslinien

Zusammenfassung

Dieses Kapitel dient dazu, Konzept und Vorgehen der klassischen Sexualtherapie aus heutiger Sicht zusammenzufassen und die verschiedenen Entwicklungsphasen, die sich seit der Grundlegung durch Masters & Johnson im Jahr 1970 herauskristallisiert haben, zu beschreiben und zu bewerten. Dabei werden auch die wichtigsten Kritikpunkte in den Blick genommen, die gegen das klassische Modell vorgebracht worden sind. Ziel ist es, dem Leser eine Übersicht zum State-of-the-Art der klassischen Sexualtherapie zu geben und die vorhandene empirische Evidenz zu ihrer Wirksamkeit und zu prognostischen Faktoren zu sichten. Daraus sollen Schlussfolgerungen gezogen werden zum Stellenwert der klassischen Sexualtherapie in unserem neuen Ansatz. Entsprechend seinem Hauptfokus wird dieser Beitrag vorwiegend Bezug nehmen auf die in der internationalen Literatur vorhandenen sexualtherapeutischen Standardwerke, Reviews und maßgeblichen Arbeiten zu Outcome, Prognose und Stabilität der Effekte der Sexualtherapie, die im Einzelnen im entsprechenden Abschnitt aufgeführt werden.

    Diese Ausführungen befassen sich ausschließlich mit den sexualtherapeutischen Konzepten und Modellen, die sich in irgendeiner Form auf das Programm von Masters und Johnson beziehen bzw. als Weiterentwicklungen oder Modifikationen verstehen. Ansätze anderer Therapieschulen finden sich zwar gelegentlich in der Literatur (z. B. Richter-Appelt (2001) zur Psychoanalyse), sind jedoch empirisch nicht überprüft und haben in der klinischen Praxis allenfalls eine marginale Bedeutung

    Da der Bericht der entsprechenden Kommission für die dritte Konsensuskonferenz (2009) nur als Zusammenfassung vorliegt (McCabe et al. 2010) und bezüglich der Effektivität der Sexualtherapie keine neue Datenlage bietet, wird er hier nicht weiter berücksichtigt.

    Der englische Begriff „crucible“ hat eine Doppelbedeutung und wird auch von Schnarch so als Sinnbilder für sein Therapiemodell verwendet. Crucible bezeichnet einerseits in der Metallurgie einen Schmelztiegel, in dem „heiße Prozesse“ ablaufen können, ohne dass das Gefäß damit verschmilzt. Die zweite Bedeutung lässt sich mit „Feuerprobe“ übersetzen.

    Der Begriff „Syndyastik“ geht nach den Ausführungen der Autoren auf ein vom antiken Denker Aristoteles benutztes griechisches Wort zurück, das übersetzt etwa „disponiert zur Zweierbeziehung“ bedeutet (Beier und Loewit 2004, S. 2).
