Abstract
Triphalangeal thumb is defined as a long thumb that has three phalanges. A thumb with three phalanges, regardless of their shape, is considered a rare congenital difference. The condition may be isolated or associated with thumb polydactyly (Fig. 24.1). In the isolated type the extra phalangeal component may have a deltoid shape that leads to angular deformity of the thumb at the interphalangeal joint. The additional phalanx varies from a small ossicle to a fully formed bone and in some cases there is no joint between the additional and adjacent phalanges [1–3]. When clinodactyly is present, the deviation of the thumb is always ulnar and this occurs primarily through the middle phalangeal segment.
