Use of Biologics in the Postoperative Management of Crohn’s Disease

Abstract

A significant portion of patients with Crohn’s disease will require surgical resection at some point in their life. Postoperative recurrent Crohn’s disease occurs in the majority of patients and is often clinically silent. Treatment options shown to modestly reduce the risk of clinical and endoscopic recurrence include antibiotics, aminosalicylates, and immunomodulators. Similarly, antitumor necrosis factor agents have been shown to reduce recurrent disease and perhaps alter the natural course of Crohn’s disease. This chapter reviews the burden of postoperative Crohn’s disease recurrence, the impact of various treatment options with a focus on biologic therapy, practical treatment approaches, and lastly avenues for future research.
