Abstract

Reactive arthritis/Reiter’s syndrome is an autoimmune condition classically manifesting as a triad of arthritis, urethritis, and conjunctivitis following an infectious illness. Anterior uveitis has been described in 3–12 % of affected patients. The antecedent infectious illness often involves the urinary tract, urethra, or the bowel. HLA-B27 is present in approximately 30–50 % of patients who develop reactive arthritis. The conjunctivitis is typically bilateral and mild; this often resolves without treatment. Patients with iritis are treated with topical corticosteroids along with cycloplegic agents. In those who have chronic or frequently recurrent intraocular inflammation, oral NSAIDs are often used. Patients with recalcitrant inflammation despite oral NSAIDs are treated with Methotrexate, and those who fail methotrexate are treated with TNF alpha inhibitors.
