Abstract

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is a rare inflammatory rheumatic disorder that is most common in elderly individuals. It is characterized clinically by an abrupt onset of polyarthritis and subcutaneous edema affecting the dorsum of the hands and less frequently feet in patients seronegative for rheumatoid factor. An excellent and prompt response to low-dose glucocorticoids is characteristic of the disease. The main issues arising when a diagnosis of RS3PE is considered include distinguishing this condition from other more common rheumatic disorders that can present with similar symptoms such as polymyalgia rheumatica or late-onset rheumatoid arthritis and establishing whether or not the patients have associated malignancies. A low threshold for appropriate screening by age and gender is required in case an underlying malignancy is not evident at the time of clinical presentation. In this chapter, we review the major features of RS3PE, with emphasis on the clinical manifestations and the diagnosis.
