Abstract
Liver transplantation is now the standard of care for children with end-stage liver disease, an increasing list of metabolic diseases and, more recently, liver tumours. It can also be used as pre-emptive treatment for some infants with metabolic diseases, where the enzyme defect resides in the liver without other structural liver dysmorphology. This chapter describes liver transplantation as it is conducted at a centre for paediatric liver disease. It is important to emphasize that liver replacement as treatment should be conducted in a setting of a multidisciplinary team specializing in all aspects of paediatric liver disease. Indications, assessment before surgery, surgical technique of donor organ procurement, split liver transplant, living donation and recipient liver explantation and insertion, are presented. Post-operative care including management of immunosuppression, post-transplant infections, surgical complications and long-term outcome are presented in detail.
