Laparoscopic Resection of Choledochal Cyst with Hepaticojejunostomy

Abstract

Choledochal cysts (CDCs) are believed to represent a risk factor for the development of neoplasia; the classical clinical triade is a right upper quadrant mass, associated with pain and jaundice. Therefore therapy consists of a complete choledochal cyst excision early after diagnosis, usually beyond 3 months of age. A resection of a CDC is an advanced endosurgical procedure, in which care should be taken not to injure the right hepatic artery or an accessory branch and the portal vein when circumferentially dissecting the cyst. The reconstruction includes a biliary-enteric anastomosis, either as hepatico-duodenostomy or hepatico-jejunostomy with a Roux-en-Y loop.
