Abstract

Historically, study of the pathology and physiology of the speech areas on the frontal convexity, and the analysis of patterns of motor aphasia resulting from damage to these areas, have formed the basis for our understanding of processes underlying speech production. However, from the earliest days of research on central speech disorders there have been hints that regions on the mesial surface of the frontal lobes played an equally important role. This paper reviews current knowledge of the functions of these regions, the anterior cingulate gyrus (AC) and the supplementary motor area (SMA), from the standpoint of the clinical effects of brain pathology. The different forms of speech disturbance and action disorder observed with lesions of these areas, seizure discharge or electrical stimulation, provide data relevant to a theory on the microtemporal processing of an action sequence, according to which AC and SMA mediate early stages in the entrainment of action components.
