Abstract
Radiosynovectomy is the local intraarticular injection of radionuclides (Yttrium–90, Rhenium-186, P-32, Erbium-169), which are thought to be rapidly phagocytosed by synoviocytes, and then distributed within the surface of the synovium causing vasculitis and occluding the microvascular supply. This, in addition to the cytotoxic effect of radiation, results in ablation of the synovium. This procedure needs only 30 % coagulopathy among hemophilics. With the overall improvement of 80−92% in most studies, it must be the procedure of choice for hemophilics with repeated bleeding and in those where surgery cannot be done. It should be considered for hemophilics with joint disease not responsive to replacement therapy and for those whom surgical synovectomy pose too many risks.
