Zusammenfassung
Störungen der Bildung der V. cava inferior (VCI) in der Embryonalzeit und Thrombosen der VCI führen sowohl zu einem akuten als auch einem chronischen Symptomenkomplex. Dieser ist geprägt durch eine akute Gefährdung des Patienten durch eine massive Lungenembolie und die rasche Entwicklung eines schweren postthrombotischen Syndroms (PTS). Die alleinige konservative Therapie mittels Antikoagulation und Kompression führt nach Literaturangaben bei bis zu 30 % der Patienten zur Entwicklung eines schweren PTS. Dies ist auch deshalb nicht akzeptabel, weil hiervon überwiegend junge Patienten betroffen sind. Nach Thrombektomie mit obligater Fistelanlage entwickelten 4 % der Patienten ein schweres PTS, nach VCI-Ersatz traten keine schweren PTS auf. Sowohl die venöse Thrombektomie, als auch der Ersatz der VCI können das Risiko eines schweren PTS bei streng selektierten Patienten vermindern.
Abstract
Deep vein thrombosis and agenesis of the inferior vena cava (IVC) in the embryonic stage of development cause acute and chronic symptoms. There is a high risk for pulmonary embolism and the rapid development of severe postphlebitic syndrome (PPS). The results of conservative treatment by anticoagulation and compression alone are unsatisfactory and according to the literature up to 30 % of these patients develop severe PPS. This is also not acceptable due to the young age of the patients involved. Thrombectomy with construction of an arteriovenous fistula reduced the risk of developing severe PPS to 4 % of patients and patients with replacement of the IVC showed only mild symptoms of PPS. Thrombectomy and replacement of the IVC appears to prevent severe PPS in a strictly selected group of patients.
