多媒體影音光碟教學對改善長照機構照護人員在導尿管相關泌尿道感染照護知識、動機及行為技能之成效

摘要


本研究旨在探討多媒體影音光碟教學介入後,在長照機構照護人員導尿管相關泌尿道感染照護知識、動機及行為技能的改變程度。研究採前後測控制組之類實驗設計,採立意取樣,以高雄縣市24家獨立型養護中心、1家醫院附設之護理之家的護理人員與照服員共144位,其中75位為實驗組(含護理人員37人及照顧服務員38人),69位為控制組(含護理人員35人及照顧服務員各為34人),實驗組個案接受為期一個月的多媒體影音光碟教學,控制組則無接受多媒體影音光碟教學。兩組人員均接受導尿管相關泌尿道感染照護知識、動機及行為技能量表之前測,一個月後進行相同問卷之後測。研究資料以SPSS14.0中文版套裝軟體進行分析,以描述性統計、卡方檢定、配對t 檢定、獨立樣本t 檢定進行統計分析。 研究結果發現多媒體影音光碟教學介入後,實驗組全體人員(含護理人員及照服員)在知識前測與後測的改變有顯著差異(p<.01),無接受多媒體影音光碟教學之控制組全體人員(含護理人員及照服員)在知識前測與後測的改變則無顯著差異,且在後測,實驗組護理人員較控制組護理人員在知識程度的改善有顯著差異(p<.01),在後測,實驗組照服員較控制組照服員在知識程度的改善亦有顯著差異(p<.01);在動機方面,有接受多媒體影音光碟教學之實驗組全體人員在前測與後測的改變有顯著差異(p<.01),無接受多媒體影音光碟教學之控制組全體人員在動機前測與後測的改變則無顯著差異,且在後測,實驗組護理人員較控制組護理人員在動機程度的改善有顯著差異(p<.01),在後測,實驗組照服員較控制組照服員在動機程度的改善亦有顯著差異(p<.01);在行為技能方面,有接受多媒體影音光碟教學之實驗組全體人員在前測與後測的改變有顯著差異(p<.01),無接受多媒體影音光碟教學之控制組全體人員在行為技能前測與後測的改變則無顯著差異,且在後測,實驗組護理人員較控制組護理人員在行為技能程度的改善有顯著差異(p<.01),在後測,實驗組照服員較控制組照服員在行為技能程度的改善亦有顯著差異(p<.01)。 研究結果顯示多媒體影音光碟教學可增加長照機構照護人員導尿管相關泌尿道感染照護知識,提升照護人員動機及行為技能,建議未來能在長期照護機構推廣多媒體影音光碟教學,以促進長照機構照護人在導尿管相關泌尿道感染照護的知識、動機及技能,進而改變在導尿管相關泌尿道感染照護的行為。

The purpose of this study is to understand the changes in catheter-associated urinary tract infection (CAUTI) knowledge, motivation, and skill in long-term care personnel after an intervention of multimedia teaching. This quasi-experimental study had been conducted in total 144 nurses and nurse assistants of 24 assisted living facilities and 1 nursing homes located in Kaohsiung city. Purposive sampling was used, and long-term care personnel were unequally assigned to either an experimental or a control group. The experimental group were 75 of them including 37 nurses and 38 nursing assistants , the control group were 69 of them including 35 nurses and 34 nursing assistants .The long-term care personnel of the experimental group had received of multimedia teaching for one month; however, the control group had not received any intervention during this time period. The long-term care personnel in these two groups both had measured by the same questionnaires before and after the one month period. The data were analyzed by SPSS 14.0: descriptive statistics, chi-square tests, paired t-test, independent t-test. Result showed: in the experimental group, the difference between the pre- and post-intervention in CAUTI knowledge was significant (p<.000); however, in the control group, there was no significant change in CAUTI knowledge between pre-and post-test. Moreover, the change of the knowledge in nurses of the experimental group (p<.01) was more significant than the control group at the post-test, the change of the CAUTI knowledge in nursing assistants of the experimental group was more significant than the control group(p<.01). The difference between the pre- and post-intervention in motivation in the experimental group was significant (p<.01); however, in the control group, there was no significant change in motivation. Moreover, the change of the motivation in nurses of the experimental group (p<.01) was more significant than the control one(p<.01), the change of the motivation between pre-and post-test in nursing assistants of the experimental group was more significant than the control group (p<.01). The difference between the pre- and post-test intervention in skill in the experimental group was significant (p<.01); however, in the control group, there was no significant change in behavioral skill between the pre- and post-test. Moreover, the change of the behavioral skill in nurses of the experimental group was more significant than the control one(p<.01), the change of the behavioral skill in nursing assistants of the experimental group was more significant than the control group(p<.01). Consequently, the results show that the multimedia teaching can increase the level of the CAUTI knowledge and increase the level of the motivation as well as the skill in the long-term care personnel. Thus, the intervention of the multimedia teaching for the long-term care personnel is recommended to promote long-term care personnel's CAUTI knowledge, behavioral skills, and by means to enhance their behaviors on CAUTI care.
