明倫跨院際營養社會支持方案自2012年與中南部7-8家醫院簽約合作,針對治療期的頭頸癌病人提供7次營養品領取及提供社會支持、情感支持和衛教認知訊息。本研究以2014至2016年1028份個案服務紀錄、各合約醫院方案年度工作報告及3份年度工作討論會議紀錄及23份期中查檢表等文件資料,採用內容分析法分析合約醫院其執行過程,萃取出跨院際合作服務經驗。研究發現執行過程受各醫院工作人員穩定度、營養品提供時機點及執行方式因其在地實際狀況而有不同執行狀況。此外,工作人員分享本方案分梯次領取增加工作人員與病人互動,形成深厚信任關係,以利資源輸送並強化其與在地頭頸癌病友資源連結,亦發揮與醫療團隊間更密切合作之效益。研究團隊期本方案能為其他社福團體、民間組織帶來拋磚引玉之效,促使工作者為個案帶來更多元的服務。
Ming Lun Cross-hospital Nutritional Social Support Program has signed contracts with 7-8 hospitals in the south-central region since 2012 to provide 7 nutritional supplements and social support, emotional support and health education awareness messages for patients with head and neck cancer during treatment. This study used 1028 complete case service records from 2014 to 2016, 3 program work discussion meeting records and 23 mid-term checklists and other documents to analyze the implementation process of contract hospitals by content analysis method and extract cross-institutional cooperation service experience; In addition, the seven staff members were interviewed in a semi-structured manner to understand their experience and suggestions. The study found that the implementation process was affected by the stability of the staff of each hospital, the timing of the supply of nutrients and the way of execution due to their actual conditions. In addition, the staff shared the program to increase the interaction between the staff and the patient, which formed a deep trust relationship, and facilitated the resource transmission and strengthened its connection with the resources of the head and neck cancer patients, and also played a closer role in cooperation with the medical team. The research team's plan can bring benefits to other social welfare organizations and civil organizations, and encourage workers to bring more services to the case.
