本專案目的在於提升慢性腎衰竭患者及主要照顧者對於飲食指導的認知率,於2015 年5、6 月問卷調查統計患者及主要照顧者慢性腎衰竭飲食認知率為54.2%、護理人員飲食認知率為75.7%,現況分析後,發現問題為:缺乏適當的衛教輔具及衛教單張、團體衛教過少、護理人員飲食認知不足、缺乏相關訓練課程、衛教指導無標準化。藉由在職教育了解飲食原則,設計衛教指導標準流程,運用創新慢性腎衰竭飲食貼本教學,修訂現有的衛教單張並增設圖示、張貼慢性腎衰竭飲食指導海報於衛教園地、增加團體衛教次數。經專案策略執行後,患者及主要照顧者對飲食的認知率提升至86.8%,護理人員對慢性腎衰竭飲食的認知率提升至100%,已獲得良好的成效。
This project was aimed at promoting the recognition rate in dietary guidance for patients with chronic kidney disease (CKD) and their primary caregivers. Questionnaires completed by the patients and their primary caregivers between May and June 2015 showed that they exhibited a recognition rate of 54.2%. The recognition rate of nursing staff was 75.7%. An analysis of the questionnaires revealed a lack of appropriate health education equipment and materials, health education workshops, dietary guidance knowledge among nursing staff, relevant training courses, and standardized health guidance. Strategies to overcome these issues include providing in-service education to promote the understanding of dietary principles, designing health education guidance standard operation procedure, implementing new CKD dietary guidance booklets, revising the current health education pamphlet and adding illustrations, installing CKD dietary guidance posters on the health education board, and increasing the frequency of health education workshops. Improvements were observed after implementing these strategies. Specifically, the recognition rate increased to 86.8% among patients and their primary caregivers, and the recognition rate of the dietary needs of patients with CKD increased to 100% among nursing staff.
