Metastatic thyroid tumor is rarely seen in clinical practice. We present herein a case of metastatic thyroid tumor from hypopharyngeal squamous cell carcinoma. The case is a 57-year-old man who was treated as primary unknown neck carcinoma at other hospital. After treatment of the neck carcinoma, the primary lesion appreared at right pyrif ormis sinus site and he visited our hospital. CT showed right pyrif ormis lesion and low density of right thyroid lobe. There is no obvious metastasis lesion anywhere except in the thyroid. Pharyngolaryngoesophagectomy and total thyroidectomy were performed and it was reconstructed by free jejunum. Histopathological study of hypopharynx and thyroid revealed squamous cell carcinoma. Gross and microscopic study of hypopharyngeal carcinoma showes no invasion in the thyroid gland. Thyroid lesion is separately from hypopharyngeal carcinoma. And thyroid tumor was considered metastasis from hypopharyngeal carcinoma.
