Complications occurred after an infection of the upper respiratory tract with Streptococcus equi sp. equi and sp. zooepidemicus in a standard bred yearling. The disease was complicated by a chronic unilateral guttural pouch diverticulum empyema and a granulomatous inflammation at the pharyngeal orifice of the auditory tube. The granulomatous tissue was found directly in the opening and prevented the outflow of the mucus and inflammatory secretion as well as a successful therapy of guttural pouch inflammation. Using endoscopically guided high frequency surgery, the granulomatous inflammatory tissue could be removed in several sessions. In the following six months the inflammation did not recur.
