Abstract: Objective To investigate the imaging findings of computed tomography( CT) and magnetic resonance imaging( MRI) for primary malignant melanomas arising from nasal cavity and paranasal sinuses. Methods CT and MRI findings of 10 cases of pathologically confirmed primary malignant melanoma located in nasal cavity and paranasal sinuses were retrospectively analyzed. Seven patients underwent CT plain scan,and one patient underwent CT plain scan and contrast-enhanced CT scan. Six patients were performed MRI scan including routine MR scan,diffusion-weighted imaging and contrast-enhanced MRI. Results Among eight patients undergoing CT examination,seven were unilateral and one bilateral; tumor centers were located in nasal cavity in two cases,three in maxillary sinus,two in ethmoid sinus,and one in sphenoid sinus. All CT findings showed that eight cases were irregular shape,soft tissue density, but no calcification and cystic degeneration. And all the eight lesions demonstrated invasiveness to the surrounding tissue and destruction of the adjacent bone structure. One lesion showed significantly heterogeneous enhancement. Among the six patients with MRI examination,five were unilateral and one bilateral; the tumor center was located in nasal cavity in one case,two in maxillary sinus,two in ethmoid sinus,and one in sphenoid sinus. All MRI findings showed that the six cases were also irregularly shaped. One showed high signal on T1WI and low signal on T2WI. The other five cases showed slightly low signal on T1WI and equisignal or slightly higher signal on T2WI. All the six cases showed high signal on DWI and four cases with enhanced MRI showed moderate heterogeneous enhancement. The six lesions also showed invasiveness to the surrounding tissues. Conclusion Primary malignant melanoma of nasal cavity and paranasal sinuses is not specific on CT imaging,but sometimes can display typical signs on MR imaging. CT,especially MRI appearances can localize the tumors,and delineate the relationship of the lesions with the surrounding tissue.
