长QT综合征的最新研究进展

Abstract: 长 QT综合征 （L QTS）指具有心电图上 QT间期延长 ,T波异常 ,易产生室性心律失常 ,晕厥和猝死的一组综合征。L QTS按病因可分为获得性和遗传性两种类型。获得性 L QTS通常与心肌局部缺血 ,电解质异常和应用某些药物有关。遗传性 L QTS由编码心脏离子通道的基因突变导致相应的离子通道功能异常而引起 ,有两种形式 :常染色体显性遗传的 RWS和常染色体隐性遗传的 JL N。目前发现有致病基因 5个 ,遗传亚型 8个 ,突变位点近 2 0 0个。临床上依基因型的不同可有特异的触发因素和 ECG表现。 L QTS的标准治疗是抗肾上腺素能治疗 ,即β-阻滞剂和（或 ）左心交感神经切除术 （L CSD） ;对少数病例 ,需辅以起搏器或 ICD治疗。对高危情况下的无症状患者也应进行适当治疗
