Abstract: For the purpose of studying characteristics and significance of changes of cardiac mechanical function in acute ischemic heart failure,the systolic, diastolic and ejectional function of the heart in 8 rabbits were measured before and at different time points after ligating the left ventricular branch(LVB) of the coronary artery. The results were compared with those of rabbits that had undergone sham operation. It was shown that +dp/dtmax of rabbits with heart failure was decreased for 22.3%,18.3%,8.4%,7.4%,23.8% and 41.8%(all P<0.05)at 5,15,30,60,120 and 180 min respectively after ligating LVB. LVSP for 23.8%(P<0.05),10.7%(P<0.05),6.4%,69%,139%(P<005) and 408%( P<005) respectively. -dp/dtmax for 329%,20.0%,139%,118%,288% and 455%( all P<005),respecyively. LVEDP was increased for 3800%,2500%,2600%,2438%,7267% and 1450.0%( all P<005), respectively. Cardiac output(CO) was decreased for 04%,40%,74%,124%(P<005),234%(P<005) and 369%(P<005), respectively. The results indicated that cardiac systolic and diastolic function decreased sharply(in reducing period) after ligating LVB,then improved slowly(in compensative period),and reduced continueously (in failing period). Diastolic dysfuction was more remarkable,whereas ejectional function was gradually decreased. These results suggested that cardiac diastolic function should be mainly improved,and systolic function be protected in the early phase of obstruction of blood flow of the coronary artery. Meanwhile,revascularization for patients with acute myocardial infarction should be performed as early as possible during compensative period of cardiac function.
