Fundamento y objetivo
Estimar la prevalencia y las características del síndrome metabólico en una cohorte mediterránea de pacientes con infección por el virus de la inmunodeficiencia humana (VIH).
Pacientes y método
Estudio transversal. El síndrome metabólico fue definido según los nuevos criterios de la International Diabetes Federation (IDF).
Resultados
Se analizaron los datos de 210 pacientes, de los que 160 (76,19%) recibían tratamiento antirretroviral; la mayoría (47,14%) recibía pautas de combinación con inhibidores de la transcriptasa inversa no nucleósidos y un 22,8% estaba en tratamiento con inhibidores de la proteasa. La prevalencia de síndrome metabólico fue del 11,42% (intervalo de confianza [IC] del 95%, 6,89-15,97%). La hipertrigliceridemia (68 pacientes, 32,38%) y las concentraciones bajas de colesterol unido a lipoproteínas de alta densidad (cHDL) (98 pacientes, 46,6%) fueron los factores observados en un mayor número de casos. El índice de masa corporal (odds ratio [OR] = 1,40; IC del 95%, 1,21-1,62; p = 0,001) y la edad (OR = 1,062; IC del 95%, 1,018-1,108; p = 0,001) se asociaron de forma independiente con la presencia de síndrome metabólico.
Conclusiones
La prevalencia de síndrome metabólico en esta cohorte mediterránea de pacientes con infección por el VIH es baja. El índice de masa corporal y la edad son los principales factores asociados con este síndrome.
Background and objective
The objective of this study was to estimate the prevalence and characteristics of the metabolic syndrome in patients with HIV infection from a Mediterranean cohort.
Patients and method
Cross-sectional study performed in consecutive HIV-infected patients attended in an outpatient’s HIV clinic at the Mediterranean coast of Spain. Metabolic syndrome was defined according to the International Diabetes Federation (IDF) new diagnostic criteria.
Results
We analyzed data from 210 patients, 160 (76,19%) of whom were undergoing antiretroviral therapy. Most of them (47,14%) were receiving combinations of non-nucleoside reverse transcriptase inhibitors, whereas 22.8% of them were being treated with protease inhibitors. The prevalence of metabolic syndrome was 11.42% (95% CI, 6.89%-15.97%). Hypertriglyceridemia was present in 68 (32.38%) patients and low HDL-cholesterol levels were observed in 98 (46.6%). Body mass index [OR = 1.40; 95% CI, 1.21-1.62; p = 0.001] and age [OR = 1.062; 95% CI, 1.018-1.108; p = 0.001] were independently associated with metabolic syndrome.
Conclusions
Patients with HIV infection from this Mediterranean cohort had a low prevalence of metabolic syndrome. Body mass index and age are the main factors associated with this syndrome.

Hypertriglyceridemic waist phenotype is a risk factor for subclinical atherosclerosis in human immunodeficiency virus-infected patients

Estudio transversal. Se consideró fenotipo «hipertrigliceridemia-cintura abdominal aumentada» si la cintura era ≥ 90 cm y los trigliceridos ≥ 2,0 mmol/l (178 mg/dl) en varones y ≥ 85 cm y ≥ 1,5 mmol/l (133 mg/dl), respectivamente, en mujeres. Se utilizó el grosor de la íntima-media (GIM) carotídeo para detectar la presencia de aterosclerosis subclínica.
Se analizaron 152 pacientes, de los que 128 (84,2%) recibían tratamiento antirretroviral; el 40,7% con inhibidores de la proteasa y un 38,1% con inhibidores de la transcriptasa inversa no nucleósidos. La prevalencia del fenotipo «hipertrigliceridemia-cintura abdominal aumentada» fue del 23,6% (intervalo de confianza del 95% [IC 95%] 16,8-30,3%). Comparados con los pacientes con cintura y valores de triglicéridos normales, los pacientes con fenotipo «hipertrigliceridemia-cintura abdominal aumentada» tuvieron un mayor riesgo cardiovascular según Framingham (media [DE] de 11,09 [7,6] frente a 3,88 [4], p = 0,001) y una mayor probabilidad de presentar lipodistrofia (33,3 frente a 13,7%, p = 0,039) y síndrome metabólico (69,4 frente a 1,9%, p < 0,001). El GIM estuvo elevado en 21 (13,8%) pacientes. Los factores que se relacionaron de forma independiente con GIM elevado fueron el fenotipo «hipertrigliceridemia-cintura abdominal aumentada» (odds ratio [OR] 4,66, IC 95% 1,05-20,6, p = 0,043) y el síndrome metabólico (OR 3,74, IC 95% 1,25-11,23, p = 0,018).
El fenotipo «hipertrigliceridemia-cintura abdominal aumentada» es un factor de riesgo de aterosclerosis subclínica en pacientes con infección por VIH y podría constituir un marcador indirecto asociado a la presencia de lipodistrofia, síndrome metabólico y riesgo cardiovascular elevado.
To study the association between hypertriglyceridemic waist phenotype and the presence of subclinical atherosclerosis in human immunodeficiency virus (HIV) infected patients.
Cross sectional study. Hypertriglyceridemic waist phenotype was considered if the waist was ≥ 90 cm and triglycerides ≥ 2.0 mmol/l (178 mg/dl) in men and ≥ 85 cm and ≥ 1.5 mmol/L (133 mg/dl) in women, respectively. We used the intima-media thickness (IMT) to detect carotid subclinical atherosclerosis.
We analyzed 152 patients, of whom 128 (84.2%) were receiving antiretroviral therapy, 40.7% were receiving protease inhibitors and 38.1% were treated with non-nucleoside reverse transcriptase inhibitors. The prevalence of hypertriglyceridemic waist phenotype was 23.6% (95% confidence interval [CI] 16.8-30.3%). Patients with hypertriglyceridemic waist phenotype had higher cardiovascular risk according to the Framingham score (11.09 [7.6] vs 3.88 [4], P = 0.001) and lipodystrophy (33.3 vs. 13.7%, P = 0.032) and metabolic syndrome (69.4 vs. 1.9%, P < 0.001) were more frequent. The IMT was elevated in 21 (13.8%) patients. The hypertriglyceridemic waist phenotype is a risk factor for subclinical atherosclerosis in HIV infected patients and it is useful to detect patients with lipodystrophy, metabolic syndrome and high cardiovascular risk.
Cardiovascular risk in human immunodeficiency virus-infected patients in Spain. CoRIS cohort, 2011
2012, Enfermedades Infecciosas y Microbiologia ClinicaLa información existente sobre el riesgo cardiovascular (RCV) de los pacientes con infección por el virus de la inmunodeficiencia humana (VIH) en España en el momento actual es limitada.
Análisis de una cohorte prospectiva multicéntrica nacional de pacientes con infección por el VIH (CoRIS) entre enero de 2010 y junio de 2011. El RCV de los pacientes se evaluó con las escalas de Framingham, REGICOR y SCORE.
Se incluyeron 1.019 pacientes (76% varones, edad media 40 años) procedentes de 13 centros que representaban a 10 comunidades autónomas. El 65,4% estaba recibiendo tratamiento antirretroviral, el 36,7% con no análogos de nucleósidos, el 24% con inhibidores de proteasa (IP) (52% con atazanavir/r o darunavir/r) y el 4,6% con raltegravir. El 56,2% tenía una carga viral del VIH < 50 copias/ml. La prevalencia de factores de riesgo cardiovascular era: tabaquismo 46%, colesterol HDL (C-HDL) < 40 mg/dl 36,1%, colesterol total (C-total) > 200 mg/dl 27,8%, edad > 45 años 27,2%, síndrome metabólico 11,5%, hipertensión 9,4%, consumo de cocaína 7% y diabetes 2,9%. El tratamiento antirretroviral se asoció con concentraciones más elevadas de C-total y C-LDL, aunque también mayor C-HDL y menor cociente C-total/C-HDL; los pacientes tratados con IP potenciados con dosis altas de ritonavir tuvieron los niveles más altos de C-total y del cociente C-total/C-HDL. Según Framingham cardiovascular, coronario, REGICOR y SCORE, el 15,2, el 6,4, el 4,2 y el 3,9% de los pacientes, respectivamente, se clasificaron en el grupo de «riesgo moderado-alto».
En los pacientes con infección por VIH de CoRIS, la prevalencia de factores de riesgo modificables es todavía elevada. Las escalas habituales identifican un número relativamente reducido de personas con RCV elevado.
Current information on cardiovascular risk (CVR) in HIV-infected patients in Spain is limited.
An analysis was made of a prospective multicentre cohort of Spanish HIV-infected patients (CoRIS) between January-2010 and July-2011. CVR was evaluated using Framingham, REGICOR and SCORE equations.
The study included 1019 patients (76% males, mean age 40 years) recruited from 13 hospitals belonging to 10 autonomous communities in Spain. Almost two-thirds (65.4%) of patients were on antiretroviral therapy (ART), 36.7% with non-nucleoside analogs, 24% with protease inhibitors (PIs) (52% with atazanavir/r or darunavir/r) and 4,6% with raltegravir. More than half (56.2%) of the patients had an HIV viral load <50 copies/ml. Smoking prevalence was 46%, HDL cholesterol (HDL-C) <40 mg/dl 36.1%, total cholesterol (total-C) >200 mg/dl 27.8%, age >45years 27.2%, metabolic syndrome 11.5%, hypertension 9.4%, cocaine use 7%, and diabetes 2.9%. ART was associated with higher total-C and LDL-C concentrations, although also higher HDL-C and lower total-C/HDL-C ratio; patients receiving PIs boosted with a high ritonavir dose showed higher total-C levels and higher total-C/HDL-C ratio. According to Framingham cardiovascular, and coronary, REGICOR, and SCORE equations, 15.2%, 6.4%, 4.2% and 3.9% of patients, respectively, were classified as having moderate or high CVR.
In HIV-infected patients from CoRIS, prevalence of modifiable CVR factors is still high. Commonly used scores identify a relatively low number of patients with high CVR.
LDL-cholesterol levels in a series of HIV-infected patients
2010, Medicina ClinicaConocer la prevalencia de pacientes con infección por el virus de la inmunodeficiencia humana que no cumplen los objetivos de colesterol ligado a lipoproteínas de baja densidad (cLDL).
Estudio multicéntrico transversal de todos los pacientes con virus de la inmunodeficiencia humana que acudieron a revisión en 5 hospitales de la provincia de Málaga entre marzo y agosto de 2007. Se clasificaron en función de su cLDL deseable National Cholesterol Education Program (NCEP): grupo A inferior a 160 mg/dl (pacientes sin factor de riesgo cardiovascular [FRCV] o solo un FRCV); grupo B inferior a 130 mg/dl (pacientes con 2 o más FRCV), y grupo C inferior a 100 mg/dl (pacientes con enfermedad cardiovascular o equivalentes o con un riesgo cardiovascular [RCV] a 10 años superior al 20%). Se contrastaron los pacientes con cLDL deseable con los que no lo alcanzaban. Programa estadístico: SPSS.
Se analizó a 1.019 pacientes, de los que 232 (22,8%) tenían un nivel de cLDL no deseable. En el grupo A había 693 pacientes, en el grupo B 163 pacientes y en el grupo C 153 pacientes; no alcanzaban el cLDL deseable el 6,6, el 53,3 y el 65,0%, respectivamente (p<0,05). Los únicos factores asociados a un cLDL no deseable fueron la obesidad (OR=1,98; IC 95%: 1,14–3,46; p=0,01), el tiempo de tratamiento antirretroviral (por cada 2 años, OR=1,92; IC 95%: 1,85–1,99; p=0,02) y estar incluido en los grupos B y C (OR=16,9; IC 95%: 10,8–26,6; p=0,00001).
En más de un 20% de los pacientes de nuestra cohorte el cLDL no alcanza los niveles deseables. Los factores asociados a un cLDL no deseable fueron la obesidad abdominal, el tiempo de tratamiento antirretroviral y el estar en los grupos B y C.
To analyse the prevalence of HIV-infected patients who do not reach their target LDL-cholesterol (LDL-C) levels.
Multicenter, cross-sectional study of all HIV-infected patients on regular follow-up in 5 hospitals in the province of Malaga (March-August/07). They were classified depending on their target LDL-C levels (NCEP): group A:<160 mg/dl, if ≤1 cardiovascular risk factor (CVRF); group B: <130 mg/dl, if ≥2 CVRF; group C: <100 mg/dl, if cardiovascular disease or equivalents or CVR at 10 years >20%). A comparison between patients reaching their target LDL-C levels and those not reaching them was done. Statistic program: SPSS.
Of 1019 included patients, 232 (22.8%) did not reach their target LDL-C levels. There were 693 patients in Group A, 163 in Group B, and 153 in Group C (6.6%, 53.3 % and 65.0% respectively (p<0.05)) who did not reach their target LDL-C. Factors associated with LDL-C levels higher than target were: obesity (OR=1.98; 95%CI: 1.14–3.46; p=0.01), time on antiretroviral therapy (for each 2 years OR=1.92; 95%CI: 1.85–1.99; p=0.02), and being included in Groups B and C (OR=16.9; 95%CI: 10.8–26.6; p=0.00001).
More than 20% of the patients in this cohort did not reach their target LDL-C levels. Factors associated with high LDL-C levels were central obesity, time on antiretroviral therapy and being included in Groups B and C.
Cardiovascular risk assessment and intervention in HIV-infected patients
2009, Enfermedades Infecciosas y Microbiologia ClinicaEl aumento del riesgo cardiovascular (RCV) entre los pacientes postivos al virus de la inmunodeficiencia humana (VIH) hace indispensable adoptar medidas de prevención de la morbimortalidad cardiovascular y, para ello, se necesitan escalas de estimación de éste, entre ellas: Framingham, PROCAM y Score. Los factores de RCV (FRCV) clásicos guardan una estrecha relación con el RCV en los pacientes con VIH, pero no se conoce sí es equiparable al de la población general. Es por ello que estas escalas probablemente infravaloren el riesgo en estos pacientes. Actualmente, se recomienda aplicar las mismas estrategias que en población general, sin olvidar las singularidades de los pacientes VIH-positivos y la importancia del estado inflamatorio persistente que presentan, que podría acelerar el desarrollo de arteriosclerosis y traducirse en un aumento en la morbimortalidad cardiovascular. Es por esto que, además de los FRCV tradicionales, los marcadores biológicos de inflamación podrían contribuir a identificar a los pacientes más susceptibles de presentar un episodio cardiovascular, por lo que son revisados. También se revisan diversas técnicas para la valoración de aterosclerosis subclínica que podrían ayudar a identificar pacientes de riesgo de forma precoz.
Los cambios en el estilo de vida (dieta saludable, dejar de fumar, mantener un peso adecuado y ejercicio físico diario) consiguen, en la población general, un descenso en la probabilidad de presentar un episodio coronario de hasta el 80%. Se revisan las medidas terapéuticas tradicionales (dislipemia, hipertensión arterial, diabetes mellitus) y las particulares de la condición VIH (supresión viral, tratamientos discontinuos, etc.).
Because of the increased cardiovascular risk (CVR) in HIV-positive patients, preventive measures are essential, requiring algorithms for risk estimation, such as the Framingham risk equation, the Prospective Cardiovascular Munster Study (PROCAM) algorithm and the Systematic Coronary Risk Evaluation (SCORE) chart. Classical cardiovascular risk factors (CVRF) are closely related to CVR in HIV-infected patients but whether this risk is comparable to that in the general population is unknown. Therefore, these algorithms probably underestimate the risk in these patients. Currently, application of the same strategies as those used in the general population is recommended, without forgetting the specific characteristics of HIVpositive patients or the importance of their inflammatory status, which can accelerate the development of arteriosclerosis and lead to an increase in cardiovascular morbidity and mortality. Therefore, in addition to traditional CVRF, biological markers of inflammation could help to identify the patients most at risk of a cardiovascular event. These markers, as well as the diverse techniques for assessment of subclinical atherosclerosis that could help in the early identification of at-risk patients, are reviewed in the present study. Lifestyle changes (healthy diet, smoking cessation, maintaining a healthy weight and daily physical exercise) reduce the probability of a coronary event by up to 80% in the general population. Traditional therapeutic measures (dyslipidemia, hypertension, diabetes mellitus) and those specific to HIV infection (viral suppression, discontinuous treatment, etc.) are reviewed.
Insulin resistance in HIV-infected patients receiving long-term therapy with efavirenz, lopinavir/ritonavir and atazanavir

Se incluyó a pacientes consecutivos en tratamiento durante más de un año con efavirenz, lopinavir/ritonavir o atazanavir. La resistencia a la insulina se evaluó mediante el índice HOMA (Homeostasis Model Assessment).
Se analizó a 47 pacientes, 18 en tratamiento con efavirenz, 17 con lopinavir/ritonavir y 12 con atazanavir. Los tratados con lopinavir/ritonavir tuvieron una insulinemia más elevada que los que recibían efavirenz (p = 0,007) y atazanavir (p = 0,020), y un índice HOMA mayor que los tratados con efavirenz (p = 0,07) y atazanavir (p = 0,028). Presentaron resistencia insulínica 5 (10,6%) pacientes: 4 en el grupo de lopinavir/ritonavir, uno en el de efavirenz y ninguno en el de atazanavir (p = 0,065). En el análisis multivariante, la pauta de tratamiento antirretroviral fue el único predictor independiente de resistencia insulínica.
Una proporción apreciable de los pacientes que reciben tratamiento antirretroviral crónico presenta resistencia insulínica. Las alteraciones del metabolismo hidrocarbonado tienden a ser más marcadas en los tratados con lopinavir/ritonavir.
To assess the presence of insulin resistance in human immunodeficiency virus (HIV)-infected patients receiving long-term antiretroviral therapy.
Cross-sectional study in consecutive HIV-infected patients treated with regimens containing efavirenz, lopinavir/ritonavir or atazanavir. Insulin resistance was assessed by HOMA (Homeostasis Model Assessment).
We analyzed 47 patients, 18 on treatment with efavirenz, 17 with lopinavir/ritonavir and 12 with atazanavir. Patients treated with lopinavir/ ritonavir had higher insulinemia than those treated with efavirenz (p = 0.007) or atazanavir (p = 0.020). The HOMA index was also higher in subjects treated with lopinavir/ritonavir than in those receiving efavirenz (p = 0.07) or atazanavir (p = 0.028). Insulin resistance was found in 5 (10.6%) patients, 4 among those receiving lopinavir/ritonavir, one among those treated with efavirenz and none among subjects receiving atazanavir (p = 0.065). In the logistic regression analysis, the antiretroviral regimen was associated with risk of insulin resistance.
A substantial number of patients on antiretroviral therapy may have insulin resistance according to the HOMA index. Alterations of the hydrocarbonated metabolism appear to be more likely to occur in patients receiving regimens with lopinavir/ritonavir.
