Zusammenfassung
Bei elf Patienten wurden die Serumkonzentrationen von Amiodaron und dessen Desethyl-Metaboliten ohne und mit Gabe von Colestyramin nach einer Amiodaron-Einzeldosis von 400 mg untersucht. Zusätzlich wurde die Eliminationshalbwertszeit bei drei Patienten nach Absetzen einer Amiodaron-Langzeittherapie bestimmt. 7 1/2 Stunden nach Amiodaron (n = 11) betrugen die mittleren Serumkonzentrationen 0,42 ± 0,11 µg/ml (ohne Colestyramin) und 0,21 ± 0,14 µg/ml (mit Colestyramin, P < 0,01). Nach Absetzen der Amiodaron-Langzeittherapie (n = 3) ergaben sich Eliminationshalbwertszeiten von 23 1/2, 29 bzw. 32 Tagen (Kontrolle 35-58 Tage, n = 8). Die Ergebnisse weisen darauf hin, daß Colestyramin die enterohepatische Zirkulation von Amiodaron signifikant reduzieren kann. Bei Auftreten gravierender Amiodaron-Nebenwirkungen ist demnach von Colestyramin eine Zunahme der Elimination und eine beschleunigte Rückbildung dosisabhängiger Nebenwirkungen zu erwarten.
Abstract
In eleven patients serum levels of amiodarone and its desethyl metabolite were determined after a single dose of 400 mg amiodarone given with and without subsequent administration of colestyramine. In addition, the elimination half-life was measured in three patients after discontinuation of long-term amiodarone therapy. Mean serum levels of amiodarone at 7 1/2 hours after dosing (n = 11) were 0.42 ± 0.11 µg/ml without colestyramine and 0.21 ± 0.14 µg/ml with colestyramine (P < 0.01). Elimination half-life after discontinuing long-term amiodarone therapy (n = 3) was 23 1/2, 29 and 32 days, respectively (half-life in eight controls: 35-58 days). The results indicate that colestyramine significantly reduces the enterohepatic circulation of amiodarone. Therefore, colestyramine should be capable of accelerating the regression of dose-dependent side-effects of amiodarone by enhancing its elimination.
