Zusammenfassung

Bei einer 71jährigen Patientin mit einer seit 50 Jahren bekannten affektiven Psychose bestanden seit 1 1/2 Jahren psychische und somatische Auffälligkeiten (Agitiertheit, Umtriebigkeit, depressive Verstimmung, Appetitminderung, Diarrhöen, Gewichtsverlust). Innerhalb von 5 Tagen kam es zu nächtlichen Verwirrtheitszuständen, Wortfindungs- und Rechenstörungen. Bei der diagnostischen Klärung ergab sich eine hyperthyreote Stoffwechsellage. Szintigraphisch wurde eine multifokale Autonomie des Schilddrüsengewebes festgestellt. Unter thyreostatischer Therapie (Carbimazol, 30 mg/d) klangen die psychischen und somatischen Symptome ab. Dieser Fall zeigt, daß bei bereits bekannten psychiatrischen Erkrankungen auch somatische Ursachen psychischer Symptome bedacht werden müssen.

Abstract

A 71 -year-old woman known to have suffered from a depressive illness for fifty years developed further psychological and somatic symptoms 18 months ago (agitation, increased impulsiveness, depressive mood, decreased appetite, diarrhoea and weight loss). Within five days there occurred nocturnal confusion with disordered word finding and counting. Biochemical tests indicated hyperthyroidism and scintigraphy showed multifocal activity increase. The psychological and somatic symptoms responded to thyrostatic treatment with carbimazole, 30 mg daily. This case demonstrates that added somatic causes of psychological symptoms should be thought of in previously known psychiatric illness.
