Zusammenfassung
Bei einem 50jährigen Patienten mit komplexen ventrikulären Rhythmusstörungen (monotope ventrikuläre Extrasystolen in Bigeminus- und Triplet-Form) wurde zum Ausschluß einer koronaren Herzkrankheit eine Koronarangiographie mit Ventrikulographie durchgeführt. Dabei zeigte sich ein etwa 2-3 cm großes Aneurysma, das sich in der Systole deutlich in den rechten Ventrikel vorwölbte. Bei einer elektrophysiologischen Untersuchung konnte die früheste Erregung während der ventrikulären Extrasystolen an der septalen Grenze des Aneurysmas lokalisiert werden. Das kongenitale Aneurysma konnte somit eindeutig als Ursache der Rhythmusstörung identifiziert werden. Eine chirurgische oder medikamentöse Therapie war nicht indiziert, da bei dem weitgehend beschwerdefreien Patienten keine hämodynamisch wirksame ventrikuläre Tachykardie bestand.
Abstract
In a 50-year-old patient with complex ventricular arrhythmia (monotopic ventricular extrasystoles in bigeminy and triplet form), coronary angiography with ventriculography revealed an aneurysm of about 2-3 cm diameter that bulged visibly into the right ventricle during the systole. Electrophysiology was able to localise the earliest excitation during the ventricular extrasystoles at the septal border of the aneurysm. Hence, the congenital aneurysm was definitely identified as the source of the arrhythmia. Surgery or drug therapy were not indicated since there was no haemodynamically effective ventricular tachycardia in the patient who was largely free from complaints.
