Summary

A new anti-arrhythmic dug, dorenadarone, will become available in the foreseeable future. Phase III of its clinical evaluation has been concluded and approval by the European and US drug regulatory authorities has been applied for. Dronedarone is further development of amiodarone, the main differences between these two drugs being that the former contains no iodine, weaker lipophilia and has a shorter half-life. It has the electrophysiological properties of properties of all four classes after Vaugham-Williams. Dronedarone has been shown to be efficacious in both rhythm and ventricular rate control. It is tolerated about as well as the placebo. There is so far no evidence of a pro-arrhythmic potential or (amiodarone-typical) extracardiac side-effects. The place of dronedarone in patients with advance heart failure or impaired renal function is not yet clear.
