Wer kennt das nicht: Nach einer ersten psychotischen Episode wird der Patient mit der (leitliniengestützten und durch psychoedukative Maßnahmen vermittelten) Empfehlung entlassen, die in der Akutbehandlung wirksame antipsychotische Medikation zur Rezidivprophylaxe mindestens für ein Jahr beizubehalten. Die Empfehlung wird aber nicht angenommen und innerhalb eines Jahres – begleitet von erheblichen Belastungen des persönlichen Umfelds, Abbrüchen von Ausbildungen, Arbeitsplatz- und Führerscheinverlust – kommt es zu einer weiteren psychotischen Episode. Jetzt wird – wiederum leitliniengerecht – zur Entlassung die Empfehlung ausgesprochen, die antipsychotische Medikation bis zu 5 Jahren beizubehalten, jedoch wieder erfolglos.
