Wirksamkeit von Indacaterol bei COPD ist unabhängig von der Begleittherapie mit inhalativen Kortikosteroiden (ICS)

Einleitung:

Indacaterol ist ein einmal täglich inhalativ anzuwendender β2-Agonist und Bronchodilatator bei COPD.

Ziel:

Gesammelte Analyse zur Bestimmung der Wirksamkeit von Indacaterol in COPD-Subgruppen mit oder ohne inhalativen Kortikosteroiden (ICS).

Methode:

Daten von 4088 Patienten mit moderater bis schwerer COPD wurden aus 3 randomisierten, doppelblinden, placebokontrollierten Studien mit Indacaterol 150 und 300µg einmal täglich, Formoterol 12µg zweimal täglich, Salmeterol 50µg zweimal täglich und Tiotropium (unverblindet) 18µg einmal täglich zusammengefasst: 44% der Patienten nahmen zu Beginn der Studie ICS, 56% verwendeten keine ICS. Endpunkte nach 6 Monaten: trough FEV1, Atemnot (Transition Dyspnoea Index [TDI]) und Lebensqualität (St. George's Respiratory Questionnaire [SGRQ]). Der Anteil der Patienten mit klinisch relevantem Unterschied in TDI und SGRQ wurde als Odd's Ratio (OR) ermittelt.

Ergebnisse:

Unterschiede gegenüber Placebo (n=661 ohne ICS, n=524 mit ICS) (n=1184) (*p<0,05, **p<0,01, ***p≤0,001) bei Patienten ohne ICS („ohne“) oder mit ICS („ICS“) (†p<0,05 vs. Tiotropium, ‡p<0,05 vs. Formoterol, §p<0,05 vs. Salmeterol, p<0,05 vs. Indacaterol 150µg).

Schlussfolgerung:

Indacaterol verbessert sowohl bei Patienten mit als auch ohne ICS den trough FEV1 und die klinischen Ergebnisse nach 6 Monaten.
