Intensivtherapie des Organspenders

Voraussetzungen für eine Organentnahme sind die Feststellung des Hirntods nach den Richtlinien der Bundesärztekammer und die Einwilligung des Organspenders zu Lebzeiten oder seiner Angehörigen.

Die Zerstörung von Hypothalamus und Hypophyse führt zum Verlust hormoneller Regulationsmechanismen mit der Gefahr eines Diabetes insipidus centralis, Störungen des Temperaturhaushalts, eines Morbus Addison sowie einer zentralen Hypothyreose.

Die Überwachungsmaßnahmen richten sich nach dem Zustand des Patienten und sollten ggf. auch Verfahren wie die invasive Messung des Herzzeitvolumens oder die Echokardiografie umfassen.

Vasoplegie und Volumenmangel führen häufig zu einer ausgeprägten Hypotonie, die die rechtzeitige Gabe von großen Volumenmengen und Vasopressoren notwendig macht. Zur Volumentherapie verwendet man größtenteils Kristalloide, ggf. ergänzt durch moderne Kolloide. Bei geplanter Nierentransplantation sollten Kolloide nach Möglichkeit vermieden werden.

Bei hämodynamischer Instabilität trotz ausreichender Volumengabe setzt man Dobutamin ein, ggf. in Kombination mit Noradrenalin als Vasopressor.

Bei beginnendem Diabetes insipidus ist die frühzeitige Gabe von Desmopressin (2 – 4 µg alle 4 – 6 h) bzw. Vasopressin (0,5 – 2 U/h) als kausale Therapie indiziert.

Eine niedrigdosierte Therapie mit Hydrokortison (200 – 300 mg/d) kann die Hämodynamik des instabilen Patienten verbessern und wird insbesondere bei geplanter Lungentransplantation empfohlen.

Die Intensivtherapie des Organspenders ist auch die vorgezogene Intensivtherapie der späteren Organempfänger.
