Zusammenfassung
Bei eineiigen Zwillingsschwestern wurde eine Mucopolysaccharidose I-S (M. Scheie) beobachtet. Die Krankheit ist klinisch durch Corneatrübung, Hepatosplenomegalie, Karpaltunnelsyndrom, Flexionskontrakturen der Finger und Zehen, Herzklappenfehler, normale Intelligenz und annähernd normalen Habitus gekennzeichnet. Biochemisch liegt ein Mangel an der lysosomalen Hydrolase α-L-Iduronidase vor. Im Urin wird vermehrt Dermatansulfat ausgeschieden. Die Muskulatur ist mitbeteiligt. Im Muskel konnten elektronenmikroskopisch in interstitiellen Zellen die für eine Speicherung von Glykosaminoglykanen typischen Speichervakuolen nachgewiesen werden. Eine ursächliche Therapie gibt es nicht.
Summary
Mucopolysaccharidosis I-S (Scheie's disease) was observed in two encygotic twin sisters. The disease is characterised by opacification of the cornea, hepatospenomegaly, carpal tunnel syndrome, flexion contractures of the fingers and toes, valvar heart disease, normal intelligence and essentially normal behaviour. The biochemical defect consists of a deficiency of the lysosomal hydrolase α-L-iduronidase. Urinary dermatan-sulphate excretion is raised. The muscles are also involved: the electron microscope reveals interstitial storage of glycosaminoglycanlike substance. There is no specific treatment.
