    Distinct gene expression footprints were identified in B cell, T cell, and macrophage cell lines (Supplementary Figure 3B). Top genes of interest in ex vivo biopsy response to FOS were identified (fold-change >1.5 vs NF; P < .25) and literature review of STRING analysis (ELIXIR) targets demonstrated that targets increased in response to FOS in pediatric IBD patient biopsies were linked to inflammation and epithelial barrier integrity (Supplementary Figure 3C).18–27 Gene targets of interest (IL1β, CX3CL1, IL23A, NLRP3) were validated by RT-qPCR in 35 pediatric patient biopsies cultured ex vivo and various cell lines, cultured with fibers, demonstrating that proinflammatory markers were increased in active IBD patient biopsies in response to FOS compared with non-IBD, mostly driven by myeloid cells (Supplementary Figure 3D).

    L-lactate acted as a volume transmitter that connects the neural activity, cerebral energy metabolism and energy substrate by activating GPR81 receptors (Lauritzen et al., 2014; Morland et al., 2015). Previous studies have indicated that L-lactate helps to alleviate impaired synaptic function though activation of GPR81 in damaged brains and to promote neuron protection (Bergersen, 2015; Lauritzen et al., 2014; Lerch et al., 2014; Morland et al., 2015). Interestingly, GPR81 activation might recover the impaired synaptic function, and may thereby help to fight the development of Alzheimer’s disease (Morland et al., 2015).

    Two well-conducted randomized controlled trials (RCTs) demonstrated lactated Ringer solution (LR) to be beneficial over normal saline and one RCT demonstrated aggressive or goal-directed hydration with LR to be more beneficial compared with standard hydration with LR among patients with mild AP.47–49 In the setting of AP, LR has additional benefits because of a direct inhibitory effect on macrophage-mediated inflammation when compared with normal saline.49,50 Based on the pathophysiology of AP, fluid therapy is thought to play the most crucial role during the first 12 to 24 hours and early hydration in the emergency room is of utmost importance.46

In addition, the inflammasome is responsible for the activation of apoptosis and necrosis of hepatocytes [7,8].
