The differential diagnosis of massive hepatosplenomegaly should include chronic lymphocytic leukemia, hairy cell leukemia (HCL), prolymphocytic leukemia, and lymphoma in an aged man with lymphocytosis, thrombocytopenia, anemia, and lymphadenopathy.
