David McConkey and co-workers carried out whole-genome mRNA expression profiling on a discovery cohort of 73 muscle-invasive bladder cancer (MIBC) samples and applied unsupervised cluster analysis to the mRNA profiles. They identified three subtypes: a basal subtype and a luminal subtype, in which the upregulated genes matched those in basal and luminal breast cancers, respectively, and a subtype that they called 'p53-like', which expressed luminal markers and genes associated with activation of wild-type p53. Gene set enrichment analyses confirmed the basal and luminal subtypes and, importantly, the basal subtype was associated with metastasis at presentation and poorer overall survival. McConkey and colleagues used the differentially expressed genes in the subtypes to generate a classifier gene set and applied this to two validation MIBC cohorts; they found that the samples could be separated into basal and luminal subtypes in which the basal subtypes again had a poorer outcome.
