Introduction
L’échange plasmatique est une technique d’épuration extrarénale permettant l’élimination d’une quantité importante de plasma compensée par des solutés de substitution. Il permet ainsi l’épuration rapide de substances pathogènes de haut poids moléculaire qui ne sont pas accessibles aux techniques d’hémodialyse conventionnelle.
Le terme plasmaphérèse désigne toutes les techniques d’aphérèse qui vont soustraire du plasma de l’organisme : échange plasmatique, double filtration cascade et immunoadsorption [1]. L’efficacité de ces techniques d’aphérèse est en général transitoire du fait d’un effet rebond survenant après la séance. Il est donc nécessaire d’y associer un traitement de fond pour prévenir la resynthèse de la substance pathogène épurée. Le niveau de preuve de l’efficacité thérapeutique des échanges plasmatiques reste faible, notamment pour les indications néphrologiques. L’utilisation abusive des échanges plasmatiques pour des pathologies mal connues, pour lesquelles n’existaient pas de thérapies médicamenteuses efficaces dans les années 1970 et 1980, ont conduit à la discréditer. Depuis, quelques essais cliniques ont permis de mieux cibler ses indications. La tenue d’un registre par la Société française d’hémaphérèse depuis 1985 a permis de colliger le nombre de patients et de séances en fonction des indications médicales [2]. La place de la néphrologie est importante dans ce registre et évolue avec le temps.

Mise en œuvre des échanges plasmatiques
La réalisation des échanges plasmatiques nécessite un abord vasculaire, une machine de circulation extracorporelle et l’utilisation d’une anticoagulation pour prévenir la thrombose du circuit. Les échanges plasmatiques peuvent être réalisés par deux techniques différentes, appelées centrifugation ou filtration. Ces deux méthodes aboutissent à l’extraction du plasma mais possèdent des avantages et inconvénients inhérents à leurs caractéristiques techniques (Tableau 1) [3]. Quelles que soient les
Mode d’action
Plusieurs hypothèses sont avancées pour expliquer le mode d’action des échanges plasmatiques mais très peu de données expérimentales ont permis de le démontrer. Les facteurs impliqués sont souvent combinés renforçant par là même la complexité de compréhension physiopathologique. On peut citer comme principaux effets des échanges plasmatiques : un rôle épurateur, un rôle transfusionnel, un effet hémorrhéologique et un effet de bio-incompatibilité.
Comment faire en pratique la prescription d’échanges plasmatiques ?
Il est important, avant de débuter tout échange plasmatique, de se poser la question de la pertinence de son application pour un patient donné. L’échange plasmatique est un acte invasif et le niveau de preuve est assez variable selon les indications médicales.
Indications des échanges plasmatiques en néphrologie
Les échanges plasmatiques ont été souvent considérés à tort comme le remède « miracle » à certaines pathologies mal connues où les ressources thérapeutiques étaient inexistantes. Faute d’étude pertinente randomisée, le niveau de preuve de l’efficacité des échanges plasmatiques pour la plupart des indications néphrologiques reste faible [22]. Autres techniques de plasmaphérèse sélectives
Certaines aphérèses ont été proposées pour remplacer les échanges plasmatiques afin d’améliorer la sélectivité de l’épuration et de limiter la compensation du plasma [97].
Conclusion
Les échanges plasmatiques constituent un élément important de l’arsenal thérapeutique permettant l’épuration rapide de substances pathogènes dans différentes situations néphrologiques. Le niveau de preuve est variable d’une pathologie à une autre, mais tend à s’affiner au cours du temps grâce aux études randomisées encore trop rares.
