Rheumatoid arthritis associated with splenomegaly and neutropenia is the classic triad for FS,2 and these patients can present with diagnostically challenging febrile illness. Fevers can be secondary to the primary autoimmune disorder, to infections, or to the development of malignancy. The risk of malignancy, particularly lymphoma, is increased in patients with FS.2 In this case, it was important to be aware that hemolytic anemia is not a common finding with FS, and a low reticulocyte count
